About Commonwealth Care Alliance Clinical Group

Commonwealth Care Alliance Clinical Group (CCACG) is comprised of nurse practitioners, physician assistants, nurses, behavioral health specialists, administrative support staff, and rehabilitation therapists with expertise in the care of elders and individuals with special health care needs. These clinicians provide primary care and care coordination to the Commonwealth Care Alliance Senior Care Options members in Eastern Massachusetts in collaboration with the members’ primary care physician. Along with the Geriatric Services Support Coordinators from Elder Services, they partner with the primary care Sites to enhance the primary care model by providing home-based assessments, on-going care and care management, and facilitating necessary support services to allow members to maintain as healthy and independent a lifestyle as possible.

Office Hours

Office hours are Monday through Friday from 9 AM to 5 PM. We are closed on major holidays. Clinicians provide emergency on-call coverage 24 hours a day/7 days a week.

Directions

Mass Pike – East/West: Take Exit 17 – Newton/Watertown. Follow signs for Watertown. Continue on Galen St. until you reach Watertown Square. Follow signs for Rt. 16, Cambridge. Continue on Mt. Auburn St. approximately .5 miles. Take a right onto Walnut St. Continue on Walnut St. until it intersects School St. Cross over School St. and you will be on Dexter Ave. The first building on your right is 200 Dexter Ave.
