If you are a MassHealth recipient under 65 years of age with a severe physical disability, you may be able to enroll in Neighborhood Health Plan’s Community Medical Alliance Program or Commonwealth Care Alliance’s Commonwealth Care Connection (Special Needs Plan) and receive your primary care through our practice.
MassHealth recipients over 65 years of age may be eligible for our practice through Commonwealth Care Alliance’s Senior Care Options program.
